We complete your organization’s form when it’s within primary-care scope. Some programs (e.g., specialized occupational or immigration panels) require specific designated centers—we’ll guide you if referral is needed.

  • Vitals & measurements – BP, pulse, temp, SpO₂, height/weight, waist/height ratio
  • Head-to-toe exam by an internal-medicine physician
  • Vision screen (Snellen) ± hearing (whisper/forced whisper or referral audiometry)

  • Form completion & physician signature (with copies in English)

Add-ons if indicated or required by your form:

  • Labs (CBC, CMP, lipids, HbA1c, TSH, iron/ferritin, vitamin D/B12, urine)
  • Titers (MMR, varicella, hepatitis B) and vaccination plans
  • TB screening (IGRA blood test; results typically 1–3 business days)
  • ECG, chest x-ray, ultrasound, or DEXA via partner centers (when specified)

How to Prepare

  • Fasting: if your physical includes lipids or metabolic labs, fast 8–12 hours (water OK).
  • TB test timing: IGRA is a simple blood draw—plan 1–3 business days for results.
