We primarily manage type 2 diabetes and prediabetes. We coordinate closely with endocrinology for type 1 diabetes, complex insulin regimens, pregnancy (gestational diabetes), and advanced device therapy when needed.

- Comorbidity prevention: vaccines (flu, COVID, pneumococcal, hepatitis B, shingles by age), smoking/alcohol counseling
Coordinate Care
- Retina exam (ophthalmology), nephrology for CKD, cardiology when risk is high, podiatry for foot issues, dietitian support

- Baseline consult (30–45 min) → history, meds, goals, exam, point-of-care glucose; order labs
- Same-day labs → A1c, CMP, lipids, urine albumin/creatinine
- Results visit (20–30 min) → personalized plan: meds, nutrition/activity, targets & timelines
- Follow-ups → quick check at 4–6 weeks; fuller review at 8–12 weeks with A1c recheck (as indicated)
Targets & Tracking (personalized)
- A1c: individualized goal (often ≤7%, looser or tighter based on age/comorbids)
- BP: typically <130/80 mmHg if tolerated
- Lipids: LDL/ApoB goals based on overall CVD risk
- Kidney: urine albumin/creatinine at least annually once diagnosed
- Feet/Eyes: annual exam (earlier if symptoms)

Sick-Day & Travel Support
- Written sick-day rules (hydration, carb-containing fluids if needed, when to hold certain meds like metformin/SGLT2, and when to test ketones)
- Jet-lag and time-zone insulin/oral-med adjustments
- Fit-to-fly and medication letters; supplies checklist
Safety: When to Use the ER (don’t wait for a clinic)
- Very high sugar with vomiting, abdominal pain, fruity breath, deep/rapid breathing, severe drowsiness/confusion
- Chest pain, severe shortness of breath, focal weakness/slurred speech
- Signs of severe infection, dehydration, or rapidly worsening foot wounds
